Add an index.

Parameters

$table: The table to be altered.

$name: The name of the index.

$fields: An array of field names.

Throws

DatabaseSchemaObjectDoesNotExistException If the specified table doesn't exist.

DatabaseSchemaObjectExistsException If the specified table already has an index by that name.
